I hereby grant The Advocate Center the right to use my child's photograph, video, and/or voice recording in any and all forms of media and in all manners including, but not limited to, composite or edited forms. I understand that my child's full name will not be used, but other materials may be released to the media without further written permission.

 

Application Requirements

In addition to this application, all student applicants for Advocate After School should submit the following:

   Three writing samples written within the past year

   A copy of the student’s most recent report card

  An essay written by the student that answers the question, “Why is it important to you to attend the Academy After School?” 

Essay Word Count Requirements

6th Grade - 150 words

7th Grade - 175 words

8th - 10th Grades - 200 words

11th Grade - 250 words

12th Grade - 300 words

Application Timeline

Advocate After School applications are accepted on a rolling basis. Space is limited. We will review your application, and you will be invited to come in for a family interview to review program curriculum and expectations. Please submit your application materials to:
